Please read through the Guidelines and the Handbook and become familiar with the contents. This will assist you in assessing whether Casa Powell Family Childcare is going to be the right fit for your family. Guidelines - These are the ten basic rules / expectations Handbook - This will give you a well-rounded knowledge of my policies and style. This should be read first so that you can determine whether or not Casa Powell Family Daycare is a right fit for your family. You will be required to sign a form that indicates you have read, understand and agree to ALL the policies as outlined 2. Please read and sign all appropriate forms. I know its a lot of forms however, my license requires these forms be completed prior to admission Admission Form - Information about the child, parents and emergency contacts. Statement of Health - Casa Powell Family Childcare must obtain, for every child who enrolls, a signed and dated statement of the child's current health status which indicates the child's abilities and/or limitations to participate in a regularly scheduled child care program. This report is to be filled out by a licensed physician or other health care professional who has seen the child in the last twelve months. CO Immunization certification - This form is to accompany the 'Statement of Health' form. Medical Authorization - This form required in order for me to administer ANY medication. Colorado state law indicates that medicine (even non-prescription), diapering products, sunscreens and lotions or insect repellants cannot be administered to a child without written permission. Authorization Forms - Please read and sign the authorization forms for Emergency medical care, topical preparations and participation in activities.
